This application is used by Dr. Tyler Black, Psychiatry Medical Services Inc. Depending on the nature of our work together, information is collected under:
Private Practice: BC Personal Information Protection Act (PIPA).
Health Authority Contract: BC Freedom of Information and Protection of Privacy Act (FIPPA/FOIPPA).
2. Confidentiality & Its Limits
Information you provide is confidential and part of the medical record. However, we are legally and ethically obligated to breach confidentiality in specific situations, including but not limited to:
Child Protection: If we have reason to believe a child is being abused or neglected.
Imminent Harm: If there is a clear and immediate risk of harm to the patient or another person.
Legal Order: If our records are subpoenaed by a court of law.
3. Purpose of Collection
We collect documents to provide assessment, treatment, and administrative support. Uploading information does not constitute a new therapeutic relationship unless one already exists.
4. Security & Data Location
We use reasonable administrative, technical, and physical safeguards. Your files are encrypted in transit and at rest. Data is hosted in Montreal, Canada (Google Cloud North America-Northeast1).
